Creating teeny tiny delicate claw settings

Does anyone have any idea how one would go about creating teeny tiny settings like this by hand (if at all possible?) I would like to make a 3 head setting where the side flanking stones are as small as 2mm diameter. Any tips would be much appreciated! Thank you

Short answer is practice, (it can be be done it is P.I.A.) alternative is to buy ready made
