Creating teeny tiny delicate claw settings

Does anyone have any idea how one would go about creating teeny tiny settings like this by hand (if at all possible?) I would like to make a 3 head setting where the side flanking stones are as small as 2mm diameter. Any tips would be much appreciated! Thank you :)
